We're not in-network with Aetna, but it's simple to apply for reimbursement on prescription eyeglasses and sunglasses after purchasing. Just follow the steps below.

Complete the claim form above and submit it along with your itemized receipt to this address:

2. Attach receipt
We'll include a receipt with your package, or you can request a receipt via info@opticsoutfitter.com.

3. Submit claim form and receipt
Note: Receipts for single-vision glasses with polycarbonate lenses do not break down the cost of frames and lenses separately because they are a packaged item. If your insurance provider has questions, please do not hesitate to reach out to us!

4. Get money back
After submitting your claim, you'll typically be reimbursed within 2-3 weeks. (Depending on your provider, it can be faster or slower.)
